Flu Shots are REQUIRED for ALL CYS personnel & children/youth by DEC. 1, 2022.
Please return your flu shot documentation to your center/program or Parent Central Services by Dec. 1, 2022. Children or staff without a documented flu shot or requested waiver will not be able to attend or participate in activities after Dec. 1.
If you require a medical or non-medical waiver form, please see your center/program director/sports office or Parent Central Services. Thank you.
